Existing and prospective aged care providers of Commonwealth Home Support Program services are invited to apply for funding to support older people to live independently within their communities.
The Department of Health (the Department) is seeking applications from eligible organisations to deliver targeted Commonwealth Home Support Program (CHSP) services across specific Aged Care Planning Regions (ACPRs).
The focus of this open selection process is to target CHSP Sub-Program and CHSP service types as outlined in the supporting documentation. The profile of the targeted priorities varies across State/Territories, ACPRs, and client (special needs) groups to address identified service gaps in localised areas. Through the delivery of timely, high quality entry-level support services which takes into account each person’s individual goals, preferences and choices, and underpinned by a strong emphasis on restorative approaches, including wellness and reablement, the CHSP will help its clients stay living in their own homes for as long as they can and wish to do so.
